Once this form is completely filled out, mail it to Love Feeds OUR World, PO Box 485, Sheboygan, WI 53082-0485. Make all checks payable to Love Feeds Our World. Please be aware that Love Feeds OUR World does not send out monthly statements, so we are asking you to please send your support in a timely manner.
